**What are Forms and how do I use them?**

Forms let you collect custom information from recipients at checkout. Things like sizing, preferences, dietary restrictions, or survey responses. Just attach a form to any gift send, and recipients fill it out before they claim their gift.

**Available on:** Team plan only

**How do I add a form to a gift send?**

1.  Start a new gift send from your Goody dashboard.
2.  On the gift details page, look for the **Add a Form** option.
3.  Add the fields you want recipients to fill out: short answer, multiple choice, dropdown, and more.
4.  Finish setting up your send and continue to checkout as usual.

Recipients fill out the form at checkout before claiming their gift.

**Can I save a form to reuse later?**

Yes! Save any form as a template and it'll be ready to select on future sends. No rebuilding from scratch.

**What kinds of questions can I add?**

Pretty much anything you need:

*   Sizing (apparel, shoes, etc.)
*   Food preferences or dietary restrictions
*   Survey or feedback questions
*   Open-ended responses

**What does the recipient experience look like?**

Recipients see the form as part of the gift claiming flow and fill it out before confirming their gift. It's built right into the checkout experience, no extra links or separate steps.

Form responses are available in your Goody account on the gift batch details page.
